Select your company and role, then create a secure password to protect your report. You'll receive a unique report number that you'll use along with this password to log in later.

Fill Out the Report Form
Fill out the dynamic report form with detailed information about the concern you're reporting. The form adapts based on your answers with conditional questions.
- The page header shows "Your Voice. Our Commitment" (or in Arabic if selected)
- Answer all required questions marked with *
- Select from dropdown options where applicable
- Provide detailed descriptions in text areas
- Upload supporting files if needed (PDF, Word, Excel, Images up to 10MB each)
- You can select multiple files at once (Ctrl/Cmd + Click)
- Files are uploaded immediately when selected
- Some questions may appear based on your previous answers (conditional logic)
- All information is kept strictly confidential
